Requests for repeat prescriptions should be made in writing i.e. by completing the repeat prescription request slip, letter or note, fax or email parkprescriptions@nhs.net
Telephone requests are not permitted to eliminate the possibility of errors.
Please do not use this form to request the contraceptive pill.

It may take longer to process items that are no longer on your regular repeat list.
